The black spots on his head could be an indication of stress. One of my Igs gets black "squiggle lines" on his head when he's stressed out - takes awhile (several hours) for them to go away once the cause of the stress is removed.
The raw skin under his nose could be from him rubbing it on something, like the walls or mesh of his enclosure.
I'm sure others will post about other possibilities too - as I'm sure these aren't the only possible causes, but was what I thought of first.
Hope this helps a little!

The dark spots on his head, along with decreased appetite may mean that he's not being kept warm enough. What are the temps in his habitat? How big is his enclosure, and what is it made out of? If its not high enough for him, he'll get bored and start face-rubbing.
